AGPP, Р1 subproject, unit 1-40, SG3001, in course of
operation of a lifting facility using a vacuum lifting gear
for sandwich panels installation, workers of POLATI LLC
(s/c of CGGC) admitted a sandwich panel fall from level
+15,000 m into a fenced hazardous area at +0,000m level.
No injuries; property damage was inflicted.

REASONS
HA Defective equipment (vacuum lifting gear).
Violation of safety requirements in course of
sandwich panels slinging (one of the ends of the

Familiarize
workers
of
the
company,
contractors/subcontractors with circumstances of
the incident.
Ensure operation of lifting gear in strict
compliance with the operating manual/passport of
the manufacturing plant.
Develop an additional fall prevention method when
using a vacuum lifting gear to ensure cargo
fixation in case of a defect of the lifting gear.
Amend the MS and the PC for performance of
works using lifting facilities, with descriptions of
types of guys for particular cargo and its methods
of proper fixation to cargo.
