2.

For passage medical examination you need:
1. Vaccination certificate (or information about vaccinations from the clinic at the place
of residence) - photocopy, 1 pc.
2. 2. The result of the fluorographic examination for the last year –original, photocopy, 1
pc.
3. 3. The result of a blood test for MRI (pre-precipitation microreaction that determines
the presence of antibodies to the causative agent of syphilis in the blood) - the validity
period is 8 days.
4. 4. Photocopies (Russia - boys 1 pc., Girls 2 pc .) (Other country - 3 copies (and girls
and boys) of these documents:
- медицинского страхового полиса;
- Passport (main page and registration)
- - страхового пенсионного свидетельства.
These documents should be addressed (in accordance with the schedule) to the registry of
the State Public Health Institution “City Polyclinic No. 21” (Student) at the following
address: ul. Orenburg Tract, 95 (Universiade Village)

If you have chronic diseases , you need statement from your medical
record
If you have a complaint, you need to say to doctors (ophthalmologist,
endocrinologist, neurologist, surgeon, cardiologist, etc.) and take
certificate.
Students with disabilities receive a copy of the disability certificate
additionally.
Students, who use contact lenses come to the ophthalmologist for
examination without lenses.
Students, who use night contact lenses must have a certificate in their
hands, which is certified by an ophthalmologist, indicating visual
acuity.
The certificate is valid for six months.
You must not eat 6 hours before blood test.

9. The procedure of passing a medical examination

1. Students come to the student clinic according to the
schedule established by the institute.
2. Students receive a ticket in the terminal indicating
the order of the queue and are distributed by
volunteers to passing examination on the 3rd floor
3. Students register in the computer database of the
clinic on the 3rd floor
4. Students fill out a questionnaire to identify risk
factors for chronic noncommunicable diseases

4. A medical examination is carried out by doctors in
the preventive examinations department (3rd floor):
1) Blood test (glucose, cholesterol)
2) Gynecologist (girls)
3) Filter (measurement of blood pressure, BMI)
4) Dentist
5) Neurologist
6) Surgeon
7) Optometrist (fundus examination)
8) ECG
9) Therapist (last)

5) After the completion of the medical examination,
medical records of students remain with the
therapist.
6) Certificates issued by the therapist indicating the
physical education group are handed over to the
employee of the Department of organization of
medical and preventive work and psychological
assistance.
7) If necessary, the student is sent for additional
examination
